Canine Autoimmune Hemolytic Anemia (AIHA & IMHA) - Patrice/Neoral Increase

Thanks for the information. There's a lot there to take into my tiny brain. I am quite scared about the increase in Neoral. I talked with the pharmacist last night and he told me that in humans dosage is determined by the amount of absorption in the body. I don't know if dogs are treated differently. Darcy is still eating, barking, waging tail, etc., but she does seem to be sleeping more and her belly seems a little bloated. And, she still wants to eat dirt whenever she has the opportunity. So here I am, giving her 200 mg. Neoral split up in two doses a day and 5 mg. Prednisone. I give her PetTinic, Pepcid, and healthy, organic food. We go back to the specialist on Tuesday for a retest of PCV and Neoral absorption(the latter takes a few days to get the results).

I did send Dr. Dodds an email about Darcy's meds and am waiting for her reply.

Dr. Dodds is also worried about the Neoral dose. She suggested that I ask the specialist if we could go on 200 mg. one day and 100 mg. the next, alternating days, OR, do 150 mg. daily. She added that the prednisone dose is good, that cyclosporine works well when given with steroids. I'm calling the specialist tomorrow. This is the second day of Darcy's 200 mg/day dose, so I'm keeping a close eye on her for any signs of bad effects. Will keep you posted.

I spoke with the specialist today and told her my concerns about the increased dosage. She doesn't seem worried about it and said that it could be only for a short time on this amount. I am keeping a close eye on Darcy and so far, thankfully, she is acting fine. Hopefully, after the blood test next week we will get some good results and will be able to lower the dosage.

She may be thinking of doing this as a "loading dose." It's not uncommon to do this with some drugs. It appears that Dr. Dodds protocol has this kind of approach.

As a matter of interest, Novartis does treat atopic dogs in this manner, a higher starting dose and then when symptoms decrease, it levels out to a lower maintenance dose.

I think the side effects you will be looking for are digestive, diarrhea and nausea. The skin side effects come from duration of treatment. I read last night somewhere (a vet website) that if this is a problem in the beginning with cyclosporine, to give it with food for a few days to diminish the nausea.

If you see bloody diarrhea, you will need to call your vet. I hope that doesn't happen.
