Canine Autoimmune Hemolytic Anemia (AIHA & IMHA) - Cyclosporine, how to not vomit

Hi all, my Taylor is taking Cyclosporine. Unfortunately, they came in 100 mg geltabs for once a day perscription. With Pepcid A/C at 10mg, she is throwing up anything she eats. She is now started on Sucralfate and will wait a few days to start the Cyclo again.

What did yall do to keep dogs taking Cyclosporine from throwing up all the time??

Any help would be wonderful.

Hi Jessica,

There are many more seasoned on here that will help you but I can tell you how Clyde is taking his meds twice a day:

11:30 AM/PM Ondansetron- so he doesn't throw up
1:30 AM/PM Carafate to coat his stomach
3:30 AM/PM Cyclosporine Modified 100 mg
Prednisone 20mg
Pepcid AC-for his stomach

He was on 1/4 low dose aspirin but he was taken off that...it made him throw up because I didn't read the directions and gave it to him twice a day instead of once. Since he threw up that one day...he hasn't again once he was put on the above schedule.

Maybe the Ondansetron and Carafate would help?

Hugs and our prayers to you and Taylor,

Kathy and Clyde

Jessica,
This is a really common side effect of cyclosporine and in general it does diminish after a bit of time.

There are a few ways to deal with this. One is to use two smaller doses a day for a short period of time, or even go to a smaller daily dose for a short period of time. Your vet might be able to get you two or three days worth of 50 mg capsules. Sometimes folks donate their unused meds back to the vet and they could have a few lying around?

Another is to actually use food when you administer it for a short period of time.
It's supposed to be given without food and it is certainly better at a full dose. But if your dog is just throwing it up, it isn't doing her any good at all! It is also very expensive and it is wasteful to have her toss it up.

Why don't you try a small snack with something that is particularly filling the next dose and see what happens. Provide her with some clean water or ice chips at the same time. It's been a while since I dosed Chance but I think I used peanut butter to coat the pill followed by a small cookie.

Chance was on a very very high dose, 250mg, for about 10 days to start. I remember he acted funny, sort of like he was spaced out. I think it can make dogs feel a little odd. I made sure he was left in a quiet area for an hour or so. He would then feel better.

Why did I put him through this? Because he was going to die if he didn't start making blood again. We were nearly at the end of that process and I saw nothing else to change his situation except this drug. I stayed focused on the process and the hope that it would work. I was operating totally on faith at that point.

We are often faced with this kind of blind following in our lives. I want you to know that everyone here has first hand experience with this and is supporting you with their faith.

I am very sorry to read about Taylor's diagnosis and hope he feels better soon. How much does Taylor weigh and what other medicines is he on and the dosages.

They do make Cylosoporine in 25mg so maybe you could give him 50 mgs two times a day if he is on 100mg daily. I have read that cyclosporinge lasts about 12 hours in his system and that is why most vets suggest giving it 2x day rather then just once. Usually about 10-12 hours a part. Also have is Taylor in Pet-tinic most vet suggest this also.

Sending hugs and prayers to Taylor

Jessica:

We used to give Sheba her cyclosporine in a little bit of soft canned dog food. To help keep it down we usually gave it to her roughly two hours after eating. She came to see it as a treat and when we eventually were able to wean her off all meds she was put out and missed her "treats". Give it a little time and do a little experimenting - you'll find the right combination eventually.

How much does Taylor weigh? I'd question the vet about the once daily issue. It's best to be given every 12 hrs due to the half life of 18-19 hrs it has. This ensures proper levels of the drug are in the body at all times...plus it's easier onthe stomach and body.

Is she throwing up after taking the cyclo or also while eating afterwards? Wondering if you might be dealing with an ulcer.

Thanks so much everyone for your feedback and good wishes. Kathy, I am happy to hear of another dog taking 100 mg also and yes we are starting back on Sucralfate so I am hoping that will be the answer. I will give the Sucralfate 2 hrs before meal time and then give the Cyclo 1 hr after the Sucralfate to hopefully coat the stomach. Taylor currently takes Prednisone (10 mg tabs) by 1/2 morning and 1/4 night, Azathioprene (1/2 tab every third day), Denamarin, Cyclosporine (100 mg once daily), Soloxine, Pepcid in the morning (Sucralfate is in the PM), Pet Tinic (2.5 ml twice daily). She weighs about 29 lbs and is a Sheltie and we have been at this since about Nov. 1st. Her current PCV is around 30 and we are kinda hanging there so that is why the vet recommended adding the Cyclo.

Thanks everyone for your endless kindness and faith!! Take care.
